Referrals

Referrals to Community Connections usually come through a Needs Assessment & Service Coordination (NASC) Agency or through ACC.

If you're looking for a service, contact your local NASC or your ACC case manager to see if you qualify, or contact us directly by phoning 04 298 7703.

Day Service

• Self referral, by family/whanau, next of kin or advocacy members
• Referred by other service providers
